
The Multidisciplinary Service is an integrated Health and Social Care team, which has been developed to provide short term support and early intervention to help people remain safely in their own home. The service is aimed at people who are:
- Aged 65 years and over
- Living in the Borough of Stockton
- Currently not open to a social care professional
The service, which comprises a multi professional team, works with Health, Social Care and the VCSE (Voluntary, Community and Social Enterprise), and can provide help with issues such as:
- Functional decline/ frailty
- Social isolation
- Financial issues
- Fuel poverty
- Assist in obtaining aids and adaptations where required
- Onecall
People will receive an assessment from a Wellbeing Facilitator who will then create an individual support plan, ensuring that the client is always at the centre of the care they receive, with any support/ intervention given free during the first 6 weeks.
Where to go
Assessments take place in a persons home and, due to the holistic nature of the assessment, usually last between 1-2 hours.

Referrals into the service can be made by calling First Contact Adult Social Care on 01642 527764, or by contacting your GP. Additionally, Health professionals can make referrals using the Single Point of Access for Health (SPA), who can be contacted on 01429 522277.
