This assessment is an opportunity for you to look at how caring may be affecting your life. It looks at various areas of your wellbeing including your physical, mental and emotional health.

You can tell us what's important to you, what you value about your caring role as well as any changes that could improve your own wellbeing and quality of life.

We will also ask you about the person you care for and in which areas you provide help and support with.
