Youth Direction - Preventions Team

The Preventions Team provides a targeted youth support service to young people aged 8 to 17 years who reside in the borough and who are:

  • At risk of school exclusion
  • Involved in anti-social behaviour within their community
  • At risk of involvement in criminal behaviour

Description

The preventions Team works with other agencies in Stockton to ensure young people have a multi-agency support plan in place, giving them access to the support they need. We work closely with young people and their families to ensure support is personalised. Each young person working with the team has an identified Preventions caseworker who will maintain regular contact and deliver consistent support to help address the behavioural issues identified. The team works closely with other agencies in Stockton to identify those young people in most need of support.

If you are concerned about a child or young person in one or more of the areas listed above, please contact the team to discuss a referral to our service.

The team offers a range of support to help young people achieve improved outcomes. This includes:

  • Multi-agency support plan and package of support
  • Targeted one to one guidance
  • The opportunity to gain accreditiations or qualifications
  • Targeted group work
  • Working with partners in the delivery of local projects and courses
  • Holiday activity programmes

How to refer:

We accept self-referrals from young people/parents and carers. Please contact the team to arrange a home visit to discuss further. If you are a professional making a referral, please contact the team as we require you to follow Stockton-on-Tees Borough Council's Early Help process for referral.
