Disabled Children's Team 'TRANSITIONS'
Transitions facilitate a smooth transition between Children, Education and Social Care: Children and Young Peoples Service and the appropriate Adult Service (Mental Health, Learning Disabilities and Physical/Sensory Disabilities).
In our case the term transitions refers to the process that occurs when young people move from children's services to adult services. At this time young people go through physical, emotional and psychological developments, changes occur in roles and responsibilities within the family, with friends, carers as well as within the wider community and school. For young people with a disability these changes can sometimes be a worrying and anxious time.
Given the complexities that young people with disabilities may face during transition from children's services to adult services it is even more important that their experience of transition is positive. All services involved with young people should ensure that this happens.
