Stockton Service Navigation Project (Social Prescribing Service)
The Stockton Service Navigation Project (SSNP) is for anyone living in Stockton aged 16 plus with common mental health problems such as stress, depression, anxiety or those with long term health conditions such as diabetes.
The SSNP can help people in taking the first steps to improve their health and wellbeing by supporting them to access activities and services. This can increase confidence, control, independence, reduce stress and isolation and help people to live healthier lives.
We can also signpost and support you to access other services such as the Citizens Advice Bureau, education providers, volunteering and job clubs.
